The Cost - Effectiveness of Transcatheter Aortic Valve Replacement for Nonsurgical Candidates

چکیده
Prediction is very difficult, especially if it’s about the future Neils Bohr Life expectancy in the United States has steadily increased in the past century, from just <60 years in 1930, to just <70 years in 1960, and to just <80 years in 2010. Progress, however, has its price. The numbers of elderly and very elderly members of the population in the United States and other developed nations are increasing rapidly along with their age-related health problems. At the same time, healthcare spending has risen dramatically. As new medical treatments have become more expensive to discover and produce, and the incremental benefits those treatments provide have become smaller, there is a growing perception that the return on continued increases in healthcare spending may be diminishing.

Transcatheter Aortic Valve Replacement: A Review Article
Transcatheter aortic valve replacement (TAVR) is a novel therapeutic intervention for the replacement of severely stenotic aortic valves in high-risk patients for standard surgical procedures. Since the initial PARTNER trial results, use of TAVR has been on the rise each year. New delivery methods and different valves have been developed and modified in order to promote the minimally invasive p...

BACKGROUND Transcatheter aortic valve implantation (TAVI) has become an alternative to surgical aortic valve replacement (sAVR) for patients at high risk for surgery. OBJECTIVE To evaluate the safety, effectiveness, and cost-effectiveness of TAVI for treatment of aortic valve stenosis in symptomatic older adults. REVIEW METHODS A literature search was performed on September 6, 2011, for stu...
متن کاملHealth Services and Outcomes Research Cost-Effectiveness of Transcatheter Aortic Valve Replacement Compared With Standard Care Among Inoperable Patients With Severe Aortic Stenosis
Background—In patients with severe aortic stenosis who cannot have surgery, transcatheter aortic valve replacement (TAVR) has been shown to improve survival and quality of life compared with standard therapy, but the costs and cost-effectiveness of this strategy are not yet known. Methods and Results—The PARTNER trial randomized patients with symptomatic, severe aortic stenosis who were not can...

Certain subsets of high-risk mitral valve patients are not suitable candidates for transcatheter therapies. The objective of this report is to present a young patient with combined mitral valve and coronary artery disease to illustrate these challenges. In this report, we present a 47-year-old man with longstanding HIV infection who was referred with severe mitral regurgitation (MR) and profound...
متن کاملCosts of periprocedural complications in patients treated with transcatheter aortic valve replacement: results from the Placement of Aortic Transcatheter Valve trial.
BACKGROUND In patients with severe aortic stenosis, transcatheter aortic valve replacement (TAVR) improves survival when compared with nonsurgical therapy but with higher in-hospital and lifetime costs. Complications associated with TAVR may decrease with greater experience and improved devices, thereby reducing the overall cost of the procedure. Therefore, we sought to estimate the effect of p...

BACKGROUND Transcatheter aortic valve replacement (TAVR) is a novel technique developed in the last decade to treat severe aortic stenosis in patients who are non-surgical candidates because of multiple comorbidities. METHODS Since the technique is performed using a transvascular approach, pre-procedural assessment of the aortic valve apparatus, ascending aorta and vascular access is of param...
